Urowell Elite is a telehealth service of Urowell Health Technologies LLC, operated under the clinical direction of Khaled Fareed, MD, in affiliation with the Urology Center of Orange County, 18800 Delaware St., Suite 750, Huntington Beach, CA 92648.

Information we collect

Information you submit through our consultation intake, chat assistant, and appointment request forms (name, contact details, date of birth, state of residence, and the health concerns you choose to share).
Standard website usage data collected by our hosting and analytics providers (such as pages visited and approximate location).

How we use it

To review your consultation request, contact you, and provide care if you become a patient.
To process membership enrollment and payments. Payments are processed by Square and financing by Cherry; we do not store your card details on this website.
We do not sell your personal information.

Protected health information (PHI)

Health information you provide in the course of care is protected health information under HIPAA and is used and disclosed only as permitted for treatment, payment, and health care operations, or as otherwise required by law. Terms of Service

The content on this website is for general information and is not medical advice. No physician–patient relationship is created until Dr. Fareed or his clinical team accepts you as a patient after evaluation.
All prescriptions require a clinical evaluation. Not all applicants will qualify for treatment, and individual results vary.
Telehealth services are available only to patients located in states where Dr. Fareed holds an active medical license (currently CA, TX, FL, AZ, NV & OH).
Membership fees are billed by Square according to the plan you select. Contact us to change or cancel your membership.
If any provision of these terms is found unenforceable, the remaining provisions remain in effect.

Telehealth Consent

Telehealth involves the delivery of health care using electronic communications. It has benefits (convenience, access) and limitations (no in-person physical examination).
You may decline telehealth and request an in-office visit at any time: book an in-office appointment.
Electronic systems can fail. If you do not receive a response to a request within 24 hours, call us at (714) 716-5959.
Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911.
